Immunization Requirements for School (Kindergarten-First Grade)

When a child enrolls in a school corporation, it is a state requirement that the parents furnish a written statement of the child's immunizations. The statement shall show: that the child is immunized against diphtheria, whooping cough, tetanus, measles, rubella, polio, and hepatitis B. This must show dates of immunizations.

Minimum Requirements for Student Grade One or Below:

  • Five (5) doses of diphtheria-tetanus-acellular pertussis (DtaP), diphtheria-tetanus-Pertussis (DPT), or pediatric diphtheria-tetanus vaccine (DT) or 4 doses are acceptable if the fourth dose was administered on or after the fourth birthday.
  • Four (4) doses of either polio vaccine (OPV) or inactivated polio vaccine (IPV), in any combination of 3 doses of all OPV or all IPV is acceptable if the third dose was on or after the fourth birthday;
  • 2 doses of measles (rubeola) vaccine, on or after the first birthday;
  • 1 dose mumps vaccine, on or after the first birthday;
  • 3 doses of hepatitis B vaccine
  • One (1) dose of Varicella (Chicken Pox) on or after the first birthday or record of disease. Parent history of chicken pos disease is acceptable as proof of immunity (no vaccine needed). A written statement from the parent/guardian indicating dates of disease and signed is all the documentation needed. Documentation by a physician is not necessary;
  • An official Birth Certificate with the State Seal will need to be presented to the school in order to verify birth date.

Your child's physician or the Morgan County Board of Health in Martinsville, Indiana. You MUST call and make an appointment (765) 342-6621.

MRSA Staph Infections

We have included below an information sheet on the MRSA Staph Skin Infections that we received from the state. With the few outbreaks in schools across the state, we feel it is prudent to remind our students the importance of good hygiene. If you have any questions, you may contact our school nurse Mrs. Blevins or Mrs. Gibbs at the high school. Both of the files below are pdf's and you must have Adobe Acrobat Reader in order to view these files.

Prescription Medicine

Parents may bring prescription medication to school for the school nurse to administer. The following procedures are to be followed:

  • Medication must be sent in the original bottle.
  • Parents should send no more than one week's supply. The bottle will be returned at the end of the week.
  • Parents must either bring the medicine to school or send a note, dated and signed, giving school staff permission to administer medication.
  • Students are not to carry medicine with them while at school or on the bus. All medicine will be stored in a locked cabinet in the nurse's office.

Non-Prescription Medicine

The school does not provide medication. If students need to take an over-the-counter medication, parents need to bring the medication to school in the original bottle, written directions for medicine administration, and complete a permission form. Any unused medication must be picked up by the parents at the end of the school year.
